Through our Health Information Organizing Package™, we’ll help you organize all of your health information essentials, both to meet emergency needs and the need for ongoing organization of your healthcare information. Comprised of four key components, and created by our nurse founder, Sue Montgomery, this unique system will be customized to your individual wishes.

The STATKey™ System provides emergency personnel who are equipped with computers immediate access to all of your important health and emergency contact information. Sue first developed this system to provide herself with peace of mind. Since her mom is an active senior who drives and and is independent at home, Sue wanted to make sure there was a system in place for emergency personnel to have access to her mom's healthcare information in case her mom wasn't able to provide it and Sue wasn't present. This unique system is comprised of two main components:

» The STATKey™ is a flash drive loaded with a template Sue created for your health and emergency information. Our professional nurse organizers will customize your STATKey™ with all of your essential health and emergency contact information, and save it in an Adobe PDF format so it's accessible to those you want to have read it. It's up to you to decide what information you'd like to have included; as much or as little as you want. We'll bring all of our own technology to your visit. You don't even need to own a computer. And, it's important for you to know, that we work directly on the flash drive and don't save any of your health/personnel information in any format. We can also scan important documents such as Living Wills, Do Not Resuscitate Orders and Designation of Health Care Surrogate forms into your STATKey™, so they’re immediately available. If a crisis occurs, emergency personnel who are equipped with laptop computers will be able to immediately access the essential information that can help them care for you in the manner you deserve. Since your emergency contact information will also be available, they'll be able to notify your loved ones to let them know what has happened, and where to find you.
» The STATKey™ Notification Card is a wallet card that serves to notify emergency personnel that you have a STATKey™ loaded with your complete emergency and health information, and lets them know where to find it. Put it in your wallet so anyone who cares for you in an emergency will know where to quickly find the information they need.
The Health Information Notebook™ is built from the information on your STATKey™. This is a portable printed file for your health information that you can take with you to doctors’ appointments to help you stay organized, informed, and in charge of your healthcare. In addition to your health history, a list of your medications, and a section for recent lab and test results, The Health Information Notebook™ will be customized to contain a separate section for each of your physicians. Each physician section will contain a blank Organize the Physician Visit Form™ that Sue designed to help guide you through your visit. This will help you to be prepared, stay focused, and make informed decisions about your healthcare. By using the Organize the Physician Visit Form™, you'll be better able to prepare for upcoming doctors' appointments, and leave with a clearer understanding of what you'll need to do next, why, where, and when. A calendar will be included to help you keep track of your health care schedule. We’ll also give you extra copies of your health history and medication lists to give to the staff in your physicians’ offices. Hopefully, you'll never need to fill out another health history or medication sheet again!
The Health Information Filing System™ is your home-based health information filing system that we’ll customize to fit your needs. Since The Health Information Notebook™ is meant to be a portable printed file for your information, the home-based system will be the permanent filing system to which information from the notebook is transferred, as well as any other health information documents you need to keep.
Professional Organizer services are included within the Health Information Organizing Package™ price. If you own a computer, and would like to learn, we’ll teach you how to update and maintain your new health information system on your own. Or, we can provide ongoing maintenance scheduled at your convenience.
